Applying to Five Branches (DAOM)

Once you have made the decision to enroll in the Postgraduate DAOM Program you are ready to begin the application process. With the MTCM or DTCM degree, you are also eligible to apply to our postgraduate DAOM program. To be considered for admission to Five Branches University, the following items are required:

Use your full legal name on all admissions documents. You may access the form here.

One recent passport-style photograph 

One-page, typewritten 

Updated to present 

Copy of Master’s or professional doctorate level diploma in TCM, Oriental Medicine, or Acupuncture 

(if applicable) 

Copy of valid malpractice insurance with Five Branches University listed as Additional Insured.

Non-refundable $65 application fee, payable by check, credit card or money order to Five Branches University

Supporting documents-sent separately:

    1. Two Letters of Recommendation sent directly to Five Branches University 
    2. Official transcripts for all AOM and healthcare-related college/university coursework, enclosed in university-sealed envelopes and mailed directly to Five Branches University
